35M / 401 Total T / 48 Estradiol. Surprised by T but worried about E2
Looking for some feedback before my telemedicine consult next week.
\*\*Background:\*\*
I’m 35 and dealing with pretty much all the classic low-T symptoms. My historical numbers have always been low: I tested at 260 ng/dL two years ago and 350 ng/dL last year. I’m honestly pretty surprised my latest test came back a bit higher at 401 ng/dL, but now I'm definitely concerned about how high my estradiol level is. I have my appointment with TRT Nation next week. Been lifting the gym 3x week for 1.5 years now and made some progress but nothing ground breaking. 6’2’’ at 223lb and 25-27% body fat if that is a factor for the estrogen.
\*\*Most Recent Labs (May 2026):\*\*
\* \*\*Total T:\*\* 401 ng/dL \*(Range: 250 - 827 ng/dL)\*
\* \*\*Estradiol:\*\* 48 pg/mL \*(Range: \\le 39 pg/mL) — Flagged High\*
\* \*\*Hematocrit:\*\* 44.1% \*(Range: 39.4 - 51.1%)\*
\* \*\*RBC:\*\* 4.85 Million/uL \*(Range: 4.20 - 5.80 Million/uL)\*
\* \*\*AST/ALT:\*\* 28 U/L / 38 U/L
\* \*\*Fasting Glucose:\*\* 87 mg/dL
\* \*\*SHBG:\*\* 31 nmol/L \*(Note: This is from my test last year, the recent panel didn't include it)\*
I know TRT Nation tends to hand out standard 200mg/week protocols right off the bat. But since my baseline E2 is already sitting high at 48 pg/mL, I’m thinking about pushing back and insisting on starting at 100mg/week, split into two 50mg shots (Monday/Thursday). I really want to avoid blowing my E2 completely out of the water.
Does this seem like a solid plan given where my estrogen is starting out? Appreciate any insights.
