unbearable flare up right before starting new job

after half a year in this shit job market, I finally got a job offer for a job I really want. just my luck, I had a really bad flare up the night before my first day so I already had to let them know and pushed my start date.

my flare-ups happen every time I'm on my period and can be debilitating. at this point, I'm at a loss and feeling completely depleted. I know when I'm not having a flare up I'd be fine, but because it comes and goes idk what to do about work.

I really don't know what to say to my new manager because after I disclosed this info to them, they asked if I'd be able to carry out the job functions (wedding coordinator which requires long days, walking, bending).

like I said, I finally found a decent paying job after being unemployed for almost a year and idk if I'll get this opportunity again. I'm just scared bc I don't want them to rescind my offer but I'm genuinely in so much pain I can't even walk for more than 5 min.

any advice on what to do re the job? :(

p.s. physical therapy isn't helping all that much, I'm already on nerve and pain meds
