I have been trying to understand where fragmentation really happens in healthcare systems, especially across referrals and multi-site care.

Is the main issue:

• System interoperability?
• Workflow design?
• Or how different teams actually use the tools?

Would be great to hear from people who’ve dealt with this directly, what tends to break first in real-world setups?
