Help me understand how healing works

I have had a fissure for 4 months, most of which that time I had 10/10 pain until I started the nefedepine about a month ago. About two weeks after using, I really started to feel less pain, and a month later things did feel like they were maybe starting to heal. I've been using all the things for regularity and soft stools but it's a tricky balance. I have had pretty loose bowel movements, until today, and they were more normal. Soft, but bulkier and more solid than they have been in a while. I am now definitely feeling more intense pain than I have in several weeks. I don't know if that means I've opened up the fissure again, or just irritated it? I don't really understand how this healing process works. My doctor just said keep up my routing for at least 3 months and beyond. It just feels like I've backslid as far as pain goes. So frustrating.

Im so tired

Got a fissure for the first time ever last June 18. Was in so much pain and spasm that led to incomplete bowel movement. Doctor suggested psillium fiber which made things so much worse I had to go to the ER to get a fleet enema on the 23rd due to fecal impaction.

Was put under observation for 3 days and still no bowel movement (BM), even if i was taking Lactulose 15ml every day. Had my second fleet enema on the 27th to prevent another blockage. Then my third on the 29th to clear my colon and reset my BM.

I went home feeling a bit more optimistic. Doctor said i should stop the lactulose now, and eat what i used to eat (i’m currently on a low-fat diet since i just had my gallbladder removed a month ago). My routine since then has been:
- warm sitz bath thrice a day
- daflon 1000mg twice a day
- faktu ointment thrice a day
- celecoxib (only when in pain)

My BMs were so far good, pain has been reduced significantly. And then today I felt the urge to poop, and passed hard, solid stools (not big nor wide, but still solid). The moment my BM passed I can feel the sharp sting and I know my fissure got inflammed again.

Im in so much pain again and it’s so frustrating I just want to go back to work and live life normally again, but ever since my surgery nothing good has happened. I hate it so much I’ve been angry, anxious, and depressed the past days. If I go back to the ER again i swear…

I don’t even know why Im posting this. Im just so sick of this, and I’m seriously contemplating seeking second advice. I could’ve prevented the hard stool if I wasn’t asked to stop the laxative. I’ve been nothing but diligent in following instructions for conservative treatment and still relapse like this.

Now Im back to zero.

Constant bleeding?

Please tell me there are others who have experienced this. A couple nights ago I had a slightly harder than normal BM and it caused my chronic fissure to re-tear there was a slight amount of blood on the paper but not in the bowl. Since then it’s been that way, but last night there was a bit more blood on the paper and a tiny bit in the bowl (which has happened before) but what I’m more alarmed about is the fact that it’s now continuing to bleed ever so slowly, so nothing crazy, even when I’m not using the bathroom. It’s not enough to cause stains on underwear or anything like that but usually the next time I go to pee, when I use a baby wipe back there, it’s blood.

Note: I have seen a CRS, I do have a fissure and I do have pain during the BM but not after. I have nifedipine which I’ve been using although not seeing too much of a difference. I’ll admit I have been slacking on the MiraLAX lately but only because I’ve been having insane health anxiety about other things so with anxiety comes the diarrhea so I didn’t need more lax on top of that. I figured it healed up enough and I could continue without it but I guess not.

I do have a colonoscopy coming up in literally a week and am already terrified of that since it’s my first one, first time being sedated, results, etc.

Has anyone else had this type of bleeding? How do you stop it? Like I said it’s not a profuse amount of bleeding at all I’m just freaking out because it’s never happened to me like this before, usually I just wipe and it’s gone and doesn’t happen when I’m off the toilet. Advice? Normal for us with fissures?

Can an old fissure reopen, but not cause any pain? My CRS doesn't seem to believe me. Really tired.

Just what the title says. I'm 33M, I have a history of anal surgeries about 5 years ago for hpv16 condyloma. I also take a bile acid sequestrant, which causes constipation.

I've had hemorrhoids which are mostly healed now, but I'm like 99% sure I have at least one chronic fissure. When I tell my colorectal surgeon, he checks and says there's nothing there and that anal fissures *always* cause pain.

I checked the area again this morning and can see the fissure with my own two eyes. It weeps a small amount of blood and then nothing. I think when I see my CRS, the fissure is mostly healed so he doesn't think it's a problem, but then it reopens from being constipated. I'm working with my gastroenterologist on maybe switching medications. Been trying adding fiber slowly, drinking tons of water, adding fiber to diet. Basically everything, with minimal relief.

What can I do? I see my CRS again at the end of the month for a checkup. I keep insisting I need to try a calcium channel blocker topical to help the fissure heal, he is so reluctant to believe me. I can switch to a different doctor but that's going to take months to get a new appointment.

Do fissures always hurt? Mine doesn't hurt really at all. If anyone has any advice, let me know.

What was your experience with Botox?

So it pretty much numbs he muscle so it can't contract like it would, so it heals faster, is that right? Also, how long does that numbing last, and also, did the injection hurt?

Does my fissure has healed ?

On 8th June I had constipation , BM was large and hard after next day , and after 2 days , I had dripping blood from my anus and it continued for 5 days more and pain was so severe i couldn't even sit , I had booked an appointment with the doctor on 19th June , 20th was last day I had blood in stool from then on I had Diltzeam cream and doing sitz bath from then .

In the first week I had pain and was unable to sit for 2-3 hours .

I used to have mild pain till June 2 , then yesterday felt like normal .

Today it felt like I didn't have any fissure .

Has anyone had recurrence of tightening/spasming/pressure sensations years after surgery?

I had a fissure that became chronic and eventually developed into a fistula, which required surgery (fistulotomy) in November 2023. After recovering, I was fine for 2 1/2 years. (Occasionally I'd have a tiny bit of bright red blood here and there, possibly related to haemorrhoids, but never any pain/other symptoms)

Three weeks ago, after having had several BMs over two days (I commonly have 2 in the mornings, sometimes more during the day depending on what I ate or if I'm on my period). I started to experience discomfort after BMs again, lasting all day, making it uncomfortable to sit and stand. It then improved to only lasting for a few hours after BM, but it hasn't resolved. It feels very much like what I experienced when I had the fistula, except this time there aren't any other symptoms (no discharge). There's no sharp pain, bleeding or anything else, and my GP couldn't see anything - just noted that the internal sphincter was very tight and that the area where surgery had occurred was a bit red.

I started using GTN a week ago to help with reducing the sphincter spasms that might be causing the discomfort. But it's still not resolved/things aren't really changing/progressing, which has me quite anxious. I've been referred to specialists but seeing someone/getting properly assessed could take months - and not knowing what's going on/why this is happening/whether it will improve on its own or whether I'm gonna have to deal with this for months to come is really weighing on me.

Has anyone had a fissure or fistula surgery, been fine for years, and then developed this sort of post-BM spasm/discomfort without any other symptoms? If so, what was it, what helped it?

I'd really appreciate hearing from anyone who's experienced something similar, thanks!

I healed my fissure in less than a week.

I had it for over a week or two. Before that I’ve had it on and off for some years but this was the worst one and didn’t go away on its own. This is what helped it get better fast.

Eat 300mg magnesium citrate every day. This makes it soft.

15 min a day, stick ur finger in some Vaseline and stick it in. (My hand was covered in plastic wrap). You must dilate and stretch it because it’s forgotten how to stretch and relax. It’s not painful at all. When ur finger is in there, breathe deeply and relax ur body. Contract ur sphincter around ur finger and feel it, then relax it and feel how open it gets. Contract and relax a bunch of times.

That’s it.

When you go to the bathroom (only go when u actually have to) if ur legs are short use a stepper so ur body isnt tense. Then when ur using the bathroom tense up the sphincter and then relax and breathe out. It’ll come out much easier. No straining. At all.

If there’s any pain after using the bathroom use some Vaseline on a qtip and apply it. Especially overnight as that is when the body heals.

Mines healed in about 4 days.

I ate whatever I wanted. Bread. Chicken. Lamb. Eggs. Nuts. Vegetables. Fruits.

HELP: rlly bad anal pain after fingering

Hi (f20) I had sex with a random guy a week ago and he fingered my bum quite aggressively. I remember his nail feeling abit sharp and it was generally very rough and it hurt abit but I thought it’d be okay. Then he tried to force his penis into my bum but I had said no to anal sex but he tried to push it in while asking and this hurt so much and I stopped him. (I’ve had anal sex twice before with my ex and it hurt but it was okay and we used lube so I didn’t get any problems).
However since this incident a week ago, my pain got really bad a couple days ago in my bum hole and it’s still really bad. I went to see a doctor and they said it was an anal tear but I can not see anything in my bum and I have no blood or pus and it doesn’t really sting, it just hurts sooo much. The muscles around my bumhole hurts so much when I sit or stand or walk around it’s so so painful I’m so scared. It hurts when I try pee as well because I’m pushing the anal muscles too and also it’s giving me a slight dull ache in my vagina too. But this pain around my bumhole and inside hurts so fucking much please what could this be and what do i do?? Im so scared its something that wont be fully cured.

Almost 3 weeks without bleeding or glass pain, am I healing?

It’s been almost 3 weeks since I last had the sharp “glass” pain during bowel movements, and I haven’t had any bleeding during that time either.
I’ve been keeping my stools extremely soft with stool softeners, fiber, plenty of water, and being careful with my diet. I still get anxious every time I have to go because of how bad the pain used to be, but so far everything has stayed soft and pain-free.
For those of you whose fissures healed without surgery:
Did you reach a point where the pain and bleeding just never came back?
How long did you continue stool softeners or fiber after you felt better?
Were you eventually able to eat normally again without worrying about reopening the fissure?
I’m cautiously optimistic, but I’m still scared it’s going to come back.

Here we go again...

So after almost 6 years of being healed, the fissure reopened and is bleeding. I'm so demoralized and scared. I cant go through all this again. I spent the morning sobbing in bed. It's not like it was before, the pain is very minimal. BUT I AM TERRIFIED, it will get to that level again if it doesn't heal back up quickly. I guess I am back to taking stool softeners, fiber caps and not eating anything out of fear.

Anyone else have this happen after a long period of being healed?

Scar tissue sensitivity

So, I *think* I'm pretty much healed if not 100% healed but I still have some sensitivity/ minor pain. The only bit of pain is when I'm seated and I kind of roughly shift side to side, it's like I'm testing where the fissure was and has now toughened up but hurts only momentarily while moving.

I can work out no problem, sitting long term doesn't hurt, bathroom time for the most part is no longer scary, its just that intermittent kind of zing/ brief sensitivity/soreness that's never more than like a 2/10 pain, especially when shifting side to side seated to test for pain. This has lingered close to a year, it's not gotten worse, but hasn't reallyy gotten better from this point. Does anyone have experience with something like this? Assuming I should just stay on course doing what I'm doing?

Help needed struggling with fissure fromast 7 days

Ive been suffering from fissure from last 9 days, ive been using fidonal cream (Lidocaine 1.5 and Nifedipine0.3) for 6 days and noticed no effect, then later went to an piles clinic and there he suggested zandu pileave pills, and creams using it for 3 days now, i already have an autoimmue health issue (IBD), how long will it take to heal it completely, already went to a general surgeon, i couldnt go to my GI as he lives in another city and traveling now is not possible, although the fissure is not very chronic, but passing bowel has been hurting alot since today morning, as yesterday while applying the cream ive put it to deep and it got struck maybe i accidently hit a tissue inside the anal and ive pulled it out with force, its paining since then, usually i have no pain while sitting, but while BM iam facing pain, using sitz bath after every BM.

Already went to a surgeon and he prescribed me lidocaine 2% and a laxative to keep my stools soft. Any tips would be highly appreciated

Need help for Laser LiS for fissure and Laser hammaroidopexy

Guys I am 26 yr M from India dealing with haemorrhoids early grade 3 and fissure dr suggested laser LIS and Lasr hammaroidopexy but i am immensely afraid of recovery and in deep depression as I read many horror stories saying that it is brutal surgery no matter is it laser or conventional both are same traumatic these things dragging me not to take surgery from months

If you Anyone has experience of this surgery please share how was your recovery after procedure did you pass poop easily or how much pain did you felt there is it worse or how much is going to loo easier or not how much did it take to normalize ?

Please help guys your words will be life saving for me 🙏🙏

Mentally exhausted

So i made a previous post here . Long story short had laser LIS fissurectomy and haemorrhoidectomy in nov 25, recurrence first started in march 26. In may i had another fissure anterior and posterior . I was almost fully healed in june . Mid june fissures reopened and this time there was bleeding as well. Controlled the bleeding and pain for around 10 days when bleeding and pain restarted . I think the sphincter tone has increased this time it hurts apply the ointment as well.
Im so exhausted from all the self blame ive been so strict w my diet and lifestyle. Tried the pelvic floor streches as well . Anytime that i have a busy day and am not able to keep up with every step of the routine im in pain again . Its time consuming and disheartening, it feels like one step forward and 3 steps back . I dont know what to do anymore . Im exhausted and afraid .
It feels so out of my control especially bcs the days that i have issues again or worse theres factors that i cant change at all like getting my wisdom teeth removed or having to do a lot of running around for unavoidable work etc

Constant pain since 10 months postpartum

From day 1 since I gave birth I have constant anal pain. I was first diagnosed with anal fissure at 6 weeks postpartum. I have seen multiple doctors/surgeons since then. I have had so many episodes of fissure flareups in last 10 months that I cry every day and feel so sad about not been able to fully enjoy my life with my infant. Even when my fissure is healed for say 1-2 weeks and my stools are soft, I still have constant anal pain. Like my muscles in pelvic floor never fully relax. Today after 3 weeks again i had a flareup. My stool was soft bt a bit sticky in the beginning and that resulted in tear. I m on such a restricted diet and I don’t know why it keeps happening. Its external fissure towards perenium and keeps tearing at the same place. I dont strain during poop and my poop is not too thick that a normal person will have a fissure episode. I m desparate for any suggestions and what helped.

Fissures as a gay man

This is so tough. I’ve had to put a pause on my sex life for over 2 months. I can’t even get an erection because that causes my sphincter to contract. I feel like I won’t ever heal with out surgery. Botox was 1.5 months ago. Didn’t help. I tried NTG, didn’t work. I’m on nifedipine for almost a month now and barely relief. As someone who is a bottom, this has made me so depressed and angry. Everything hurts. Just a rant.
