Adult Disability Payment Application Scotland

Hi everyone, looking for some advice here.

I’m based in Scotland and applying for adult disability payment. I have Dyslexia, PTSD, Anxiety and depression which was diagnosed almost 10 years ago. About 4 years ago my therapist who I had been seeing at that point for 3 years told me he suspected I have adhd and am on the spectrum. My brother who has ADHD also agrees with that as does my friend who is also ADHD and autistic.

For years I have had to come up with ways around it to try and cope without medication including video calls or in person assistance with cooking, reminders to eat, drink water and bathe. With my depression it gets harder as any motivation to complete those tasks disappear and will only be done if my friends or mum come over to make me.

My nhs history is all over the place as I struggle to get appointments as they are always full even on the days I remember I need to call at 8am.

An advocate is helping me fill out the form but I’m wondering what other supporting documents I can use? I’ve been on and off antidepressants for years, I have an employee passport from work I could use which details workplace adjustments. I’m not sure what else to do, I was hoping if successful in obtaining ADP to then use that money to get a private adhd assessment as I’ve been on the waist list for a year already and it’s not looking like I’ll be seen anytime soon.

My mental health is getting worse each time I’ve tried to fill out the form as coming to terms with how much I reply on prompts as if I’m a child for basic needs has really hit me and made me feel like I’m a burden to those I love. My brother used to cope like me until his private diagnosis and getting medication and now he can do the basics himself and only requires prompts for more detailed tasks with lots of steps or out of the blue ones (like remembering clocks go back or MOT’s)

I’m really worried that because I keep being unable to get appointments when I need it that it will stop be being entitled and subsequently delay me even more in getting a diagnosis and help I need.

Any advice on additional information/supporting docs that I should think of would be greatly appreciated. I have my appointment with the advocate next week to look at part 2 of my application.
