Really don’t understand different lab “normal” ranges

My Free T4 is 11 pmol/L on a lab range of 9-19. Yet, many other labs have a 12-24 range, so if I just went to another lab I’d be considered “abnormal”? How does this make sense in practice/for diagnostic purposes?

Btw, I am partly asking because I am confused as to why my Free T4 is this low if my TSH is normal (even if “highish normal”… 2.31), and even on synthroid.

Any thoughts on my labs?

Been on 50 mcg levo for 7 weeks. Started at 4.14 TSH but nothing else was tested at that time. Seem to have macrocytosis despite normal b12 and folate. Also, are my t4 and t3 unusually low given my TSH now (lower since taking meds?)

TSH 2.51
T4 - 11
T3 - 1.1
Both antibodies (tpo and tg) high - 104 and 76
MCV - 103
MCH - 34

Curious about TSH and T4 blood readings

So, I have a family history of various thyroid issues (including my whole female lineage on my mom’s side having goiters and ultimately getting cancer). I have some symptoms that could point to hypothyroidism and my recent TSH reading was over 4. My doctor agree it could be a good idea to start me on low dose synthroid.

However, a year ago my TSH reading was 2.5 and free T4 was 11 (pmol/L). Whole neither was technically out of range at that time, the T4 seems like it was out of range according to how many other labs measure. Is it common for T4 to be that low with a “fairly normal” TSH reading?

They didn’t take my free T4 this time, so I don’t actually know if I am subclinical or clinical. Given my TSH is considerably higher, would it follow that my free T4 also likely dropped out of range?

I am now on meds and will get a full panel in 6-8 weeks, but of course it will be impacted by the meds. I guess I am just curious whether I am typical subclinical, or could actually be clinical.

I feel like I don’t have an objective sense of the strengths and weaknesses of my voice. Any feedback or suggestions are appreciated - trying to understand how to be more engaging/dynamic.
