What is this type of connector? (wire to PCB)

I am working on repairing the video connection on a PTZ I/O board and need to identify a connector. It's like a half-pipe shape, has 4 pins, and the connector crimps the braided shield, while leaving the center conductor free. Just as some background info, the connector is attached to RG174 coax cable, which is then soldered to the board, so it acts as an intermediary between the coax cable and the I/O board.

In the first photo you can see that the connector has 4 pins, 2 on each side, and that the pin go through the board via some though-hole points.

In the second photo you can see the bottom side of the connector. Here you can see that the first two pins are still attached but the last two pins have broken off.

In the last photo, you can see that the connector is like a half-pipe, with the bottom being open to allow the center conductor to pass through the board.

I've been searching online for the past 3 days and can't find the name for this type of connector. One detail I need to mention: the pitch between the holes in the PCB is 5~5.08mm (or .2"), but if anyone can tell me the name of this type of connector, I'll dig a bit more for the specific one I need.
