Birth certificate photocopy question

For the i-129f packet, the guide we are following shows that the gal who submitted her packet put the photocopy of her birth certificate as a smaller image on an evidence page, underneath a lable of what it was.

I got my birth certificate photocopied, and it is the entire page. Is it okay to include a page labeling it, and then attach the whole photocopy, or should I have the copy smaller on the page WITH the lable?

I also noticed, the guide shows her only copying and including the front of her birth certificate, and every source I've looked at says to include both the front and the back on separate pages.

My fiance seems to think we have to follow the guide exactly or we'll be denied. I think it'll be fine to include the copy as a whole page, with a page attached to it labeling what it is.

My fiance lives in Mexico. He has a friend who immigrated to the US via K-1 visa (the way we want to go about things), and stated that some US states require birth certificates to be apostilled for the process. Which states require this??

I've called every possible source to figure this out, and am beyond frustrated with automated systems pointing me in every wrong direction.
