Knee problems while riding

I’m in desperate need of help! I’m an Ironman athlete and recently started getting pretty bad pain on the inside of my right knee during long outdoor rides. I did a 3 hour ride two days ago, and could already feel the pain after about 1 hour, but pushed through and finished the ride anyway. Now I can barely walk normally, stairs hurt, and bending the knee is painful — although I still have full range of motion and no locking/clicking. What confuses me is that I’ve done long indoor trainer rides all winter with zero knee problems, and this has only happened outdoors. I recently had a bike fit for optimal TT position, where the fitter also mentioned wanting my cleats further back, but they’re already maxed out. Has anyone experienced something similar, especially with only one knee being affected? As far as ChatGPT says, it’s most likely my MCL, but I’m honestly not sure!
