Dandy implant crowns

Attempted to deliver my first dandy implant crown (screw retained). The scan body was verified by a PA and had complete seating.
All the scans that were sent were good.
Gingival tissue looked good. Didn’t have any blanching or any indication that it could be in the way.

Implant is from implant direct- placed by a previous associate at the practice.

Any ideas as to if this a lab issue? Or any tips/tricks for me, maybe I’m missing something?

I work for a small DSO that currently started using dandy as the main lab. My first time working with this lab.
Any inputs are greatly appreciated!

Patient wants a refund

Did a buildup and crown prep on a patient. Attempted to seat the permanent crown twice without success and scheduled patient back for a third delivery appt.
During the appt patient was very cordial and understanding and did not express any frustrations or concerns.
During the weekend patient sent angry text messages saying that they will not be returning to the practice and would like a full refund. They even called their insurance to ask for the money back.
Attempted to reach out but pt has been ghosting us.
At this point I don’t want the headache and I’m okay to refund her the money but not without her signing some type of a letter stating that we would not be liable for further treatment and also dismissing her.

My question is, where does one find such a document or are there any templates out there that could be used as a reference?
