Near Month 9 ACL Recovery — Strength Is Back But Explosiveness Isn’t

Around 9 months post-op ACL reconstruction, LET, and meniscus shaving and still dealing with quad activation issues + patellar tendon pain. Wanted to see if anyone else has gone through something similar or has advice. Right now I’m about 6’6 210lb

I’m attaching a picture of both my legs because my surgical side still looks noticeably different. My quad especially feels “asleep” a lot of the time when I wake up. I can activate it, but it still doesn’t feel fully normal or explosive compared to the other side. I also still have a tiny bit of swelling/puffiness around the knee almost all the time.

The weird part is I can do certain things with little to no pain — pogos have never really hurt, I can lift pretty heavy, do step-ups, squats, sled work, calf work, etc. My conditioning has also improved a lot recently. But when I try to get into more reactive athletic movements like bounds, triple jump landings, or hard jumping/cutting, the patellar tendon pain shows up.

I’ve been grinding rehab consistently and my PT thinks quad inhibition/activation is still a major issue. I also still have slightly more hyperextension on the surgical leg than the other one. Sometimes it honestly feels more neurological than strength-related.

For anyone who’s been through this:
- Did your quad ever feel “offline” this far into recovery?
- Did lingering swelling affect your quad activation?
- What finally helped your tendon pain calm down?
- Did PRP help anyone?
- Has anyone tried peptides like BPC-157 or TB-500 and actually noticed a difference?
- Did you eventually regain explosiveness/jumping ability?

I’m trying to get back to high-level athletics, so this has been mentally frustrating because some days I feel close and other days it feels like I’m stuck. Any advice or experiences would help.
