What is the best way to get my records corrected?

I am having an issue with my claims that the version of the DD214 the VA is sending to examiners is not my most recent DD214 and shows my service period ending 4 years before it actually did. I have uploaded the correct version of my DD214 taken it to appts with me, but even in my C&P exam the examiner stated in her medical opinion that the events did not happen durring service, even when she specifically calls out a year that was my my recent DD214 reflects that I was in fact on active duty service. The decision letter that I received also only specifically called out my years of active duty, while on deployment, and not my actual full active duty service period. I have already done one HLR where a DTA was found and kicked back, but denied again because of the service period years and the year of my initial diagnosis. So I guess I actually have two questions here.

  1. What is the best way to get my records corrected and to get the VA to look at all of the evidence?

  2. What is the best way to to get things like my TERA Memo which doesnt reflect my full in service period and exposures, as this seems to be what the VA is heavily relying on.
