Can someone refer a US physician to fill out an OSA related employment form?
I need an FAA document filled out saying my form of treatment (CPAP) is effective. Lofta offers this service however after 1 month of dealing with them they can’t even fill the dates correctly on this form and I’m up against a deadline for submitting the document, which is why I’m asking here. Any recommendations would be much appreciated 🙏🏻
