How to List Preferred Names?

I am not sure if this is technically a grammar question, but I could use some guidance. I am updating a contact list for our alumni directory. When someone uses a nickname that is a shortened version of their name, it is currently placed in quotation marks.

For example:
Last name, First name “Nickname” Middle name, Maiden name - would be:
Jenkins, Catherine “Caty” Ann Barlowe

However, we now have a few people who use their middle name as their preferred name, and I am seeing it formatted several different ways in the report I’m working from.

For example:
Last, First “Middle” Maiden
Last, First (Middle) Maiden
Last, First (Middle) Middle Maiden

What is the best way to handle this? Is there a standard or preferred format?
