Hello po! Freshie po here sa UST SHS and I am an ESC grantee po and graduated po sa private school. I’m part of Category D po. Me and my mom pati na rin po some of my classmates sa old school here in UST are confused lang po concerning the requirements na hinihingi po ng Annex E.

As I have mentioned, ESC grantee na po meee so ung proofs na hinihingi po sa Annex E ay naipasa na po dati. Tulad po doon sa affidavit of family’s financial capacity and income tax return. I’m confused lang po since UST requirements requires only SF9, PSA Birth Cert., ESC Certification, and Accomplished Annex E VP Form for CATEGORY D.

Here are some of my questions po:

- Magpapasa pa po ba ng proof of income such as ITR? Saan po ito iaattach if ever? Sa same PDF File po ba ng Annex E?

- Digital po ba iffillout ang Annex E or ipprint tas scan?

- Ung Applicant’s Information lang po ba ang iffillout? Hindi na po kasama ung Part III po ng Annex E form? Mukha po kasi siyang checklist.

- Basing on the GFORMS po for the voucher and UST’s general requirements SF9, BIRTH CERT, and accomplished Annex E lang po ang hinihingi nila. Kailangan ko pa po ba iattach ung iba pa pong general requirements na sinet po ng Annex E po mismo or no na?

Hi! Just want to ask if may bawal po ba with regards to earrings po ng girls in UST? Sa past school ko po kasi bawal ang dangling and such. Aside po sa bawal ang double piercing (?), meron pa po ba silang pinagbabawal like dangling, flashy,and mostly chinecheck ng guard or wala naman po basta single pair lang? Thank youuu!!!
