New grad here. Just curious what y’all experienced denture people think about this Tori? I’m thinking it’s probably just a bit too big to put a denture over. Idk I haven’t seen a lower denture sit on Tori this big but I’ve also never tried. Y’all think she’d need some more surgery? Is it more of a comfort thing or a seating/necessary extension thing? Don’t know why the OMS didn’t remove it… :/ new patient to me.

I tried to get the best angles for pics but if y’all need casts for it to evaluate let me know

Bridge vs RPD tx plan w/ discussion tips?

New doc here. Patient coming in soon for fillings (#4-MO, #18-O) and don’t worry 1, 15, 16 and 17 are out now. Planning on discussing replacement options for lower sites. Not exactly sure how I should present each option, or if it’s feasible/good prognosis.
For the lower right I’m concerned 31 is too mesially tipped for a bridge, and given that each would be a pretty long span 4 unit…. And her caries risk is pretty high

If she wants to do our RPD, what are your tips for explaining each? Do you take casts/scans to evaluate undercuts? I plan on presenting resin, cast, Valplast. I think this would be a very good cast metal case, obviously depending on undercuts, I think I-bars would work well since she is esthetically concerned
Speaking of esthetics, I hear valplast is ass… never done one so I don’t know, just heard of the limitations (not being able to add teeth, hard to adjust)

I will talk to her about it but I’m pretty sure implants are out of the question financially

What to do with denture??

denture delivered 2 months ago but doesnt retain. I’m a new doc at this office (1 month outta school too) so just inherited the first adjustment. Bite is good, esthetics are good except that lip is protruding. Saw her in a bind of time last week and didn’t know exactly the best move, so just soft relined it. I looked at the frenum and adjusted it slightly but don’t think that is the culprit. Seeing her in two days and not sure the best move, or if I even can reduce that lead protrusion at all. My thoughts are that maybe I can hard reline and then take away some of the anterior, because potentially it will have hard reline material added to the intaglio behind the anterior so I can afford to shave down some of the true anterior against the lip, but I guess that’s only banking on if the fit now is lacking any anterior, which probably isn’t likely.

Idk I just want to try and fix this without having to remake it. Any advice?

Itero ever give y’all this problem??

Did a crown prep and every time I scanned it came up with this extra looking flash thing. I swear the gums weren’t in the way. I know prime scan is way better but this is all we have. I did a traditional impression after this and plan on sending both to the lab. If it matters, the lens was kinda blurry and I didn’t know how to clean it at that moment. 3 weeks into practicing 🙃💯

To remove or not to remove?

Hello all, patient is like ~60F, got full mouth EXT with alveoloplasty - which she had to get redone once - like a year ago, been thru a lot before now starting denture process. Has various health issues like multiple sclerosis, edema, and she says “chronic inflammation”. She claims she went back to OS like 3 times to see about this tender spot. She says it’s been there since after they operated the 2nd alveoloplasty, but the OS is now refusing to treat her and says she’s ready to go with dentures. She says it’s mostly just annoying but sometimes flares up a bit and it’s pretty tender (she chews (gums) mostly on that side). I’m just not sure if it’s a bone spur, exposed bone, a foreign object, or what.
This is my first time meeting her (3rd week on the job), Maybe I give the OS a call and ask and potentially refer again? I don’t want to open it up and see myself. Or I could just go thru with the dentures. The patient says she might be fine without any other intervention but I’m afraid it’s gonna rub too much. Any removal of bone won’t affect retention though, she’s barely got a ridge.

Any thoughts appreciated.

Would you recommend replacing? Or how would you approach/word this?

Ngl y’all I’ve made a few mistakes recently and I’m starting to rethink everything. Just graduated so I know it can be normal growing pains, but just want some additional voices. What would y’all do with this open margin? Clinically explorer can go in but no decay felt. I informed the pt and told him long term it’s best to replace. He got it done in Jordan a year and a half ago, he said he may go back there and see if they’ll do it for discount

Need help with tx planning

17M. Medicaid. Minimal finances. Dealt a bad hand in life (on 4 anxiety meds). Collapsed VDO (not exactly sure why - I think combination of biting edge to edge and maybe something skeletal). I saw him my second day ever of practicing for a periodic exam. He is an established pt that has had a lot of restorative work here including the RCT’s on 8&9. Those have now broke off and I couldn’t think of the best tx option right then and there so I said I’ll consult with some people and get back to him. Already asked my lead doc and he gave me an opinion but curious what you random people on the interwebs think…

Able to seat some dentures?

Hey y'all, new grad here, been practicing a week lol. I'm gonna be flooding these forums with questions over the next few months just to have some additional guidance from as many opinions on the internet I can find, as well as a few mentors I've got too. So yes this patient had all remaining extractions 8 weeks ago and comes to our office like this. I don't want to make anything that really wont be comfortable for him, but he's got a decent amount of excess gingiva that you can (kinda - pur intraorals arent great) see on these pictures, particularly the mandible. Considering referring him to a periodontist for a gingivectomy, but also dont know if its necessary or how to properly decide. Any suggestions?
