25F | Large fibroid (12cm), rapid growth -- should I skip the wait and go straight to open myomectomy?

Hi everyone,

I (25F) have a large fibroid that's grown from 4cm to 12cm in just 11 months. Because of the growth rate, my gynaecologist is investigating the possibility of leiomyosarcoma.

My MRI came back inconclusive (the radiologist couldn't determine aggressivity), so my gyno is now seeking a second opinion on the MRI and ordering LDH isoenzyme testing and a PET scan to rule out sarcoma before proceeding.

He's very pro-laparoscopy, so the plan is to put me on Zoladex for 3 months prior to surgery once I get the all clear from sarcoma. All up, I'm looking at a 4 to 5 month wait (LDH isoenzyme testing is slow here in Australia, and there are wait times for PET scans too + the 3 month Zoladex lead up once I get the all clear for cancer).

I've been dealing with really debilitating symptoms from this fibroid for 5 to 6 months now and I'm at my wit's end. Every new test or appointment just adds to an exhausting waiting game.

I know my chances of sarcoma are very low given my age and I understand that the only definitive way to know is through biopsy of the fibroid itself. From what I've read, LDH and PET scans aren't particularly reliable for ruling out sarcoma anyway.

So I'm wondering should I just skip all the extra testing and waiting and go straight to an open myomectomy? I'd like to have kids in the next 1 to 3 years, which factors into the decision.

Has anyone been in a similar situation? Would love to hear from people who've navigated this kind of uncertainty.

For context: My gynaecologist does not want to go ahead with a lap until he is sure of no cancer (due to the possibility of spreading it if it is).
