Tell me about your medication errors

Hi All

I made a medication error tonight, and I feel awful about it, I can't stop beating myself up. How it happened: I gave a patient his evening meds, immediately got pulled away to get an S8 out and give that, and help with a wrist reduction, and by then I'd completely forgotten to tick off those evening meds on the medication chart. By the time I remembered, about 30-40 mins later, another nurse had come around and given the evening meds again. So I essentially caused a patient to be double dosed on his evening meds. Like I said I feel terrible, and so silly - this was such a simple mistake on my behalf. Luckily the patient will probably be fine. I don't know what I want out of posting here... I know we all make mistakes, so I guess this is a hard way to learn to tick off meds IMMEDIATELY as soon as they are given... My tail will be between my legs for a while :(
