Current CMS employees (even those who have been employees for years are now being required to complete a background investigation form (i.e., Standard Form 86) as part of the Trust Workforce Program. The form requires you to provide the citizenship information of ALL RELATIVES. This information includes the person's name, COUNTRY of birth, address, and date of birth. Be forewarned --- DHS staff have access to this information and use it to TARGET people of color for immigration enforcement REGARDLESS of citizenship. Why else would they ask for the COUNTRY of origin? If your relative is from for example Africa, the Caribbean, or a Latin American country then you should worry.
