Where to find a mental health clinic to get a shindansho (診断書)?

I’m so stressed and it’s really affecting my mental health. It’s affecting my physical health too.

On more times than I’d like to count I’ve been really tempted to take the most severe way out but I thought I would try to see if I can get time off work first.

How do I get this? Is there anything in particular I need to include in it? Unfortunately I live rural

How to quit Interac mid year?

I’m aware I need to give a month’s notice but what’s the exact process? Anything I need to watch out for/know?

If I quit will they take away my leave?

I have approved leave for the end of this month but for various reasons I want to quit and give the company a month’s notice starting from next week. If I do so though, will they take away the already approved leave I have at the end of the month? Is that allowed?
Also is it possible for the company to take away my rental car?

Can you get fired for going sick too much?

I’ve already taken a week of sick leave and we’re not that far into the school year. All were for valid reasons but now I’m worried I could be fired if I take any more sick leave, but I can’t help if I get sick? So is it possible to be fired for taking too much sick leave?

More info:
I’ve run out of paid leave already.
And I also don’t plan on staying longer than a year. My main worry is just being fired, or if there’s any other shenanigans the dispatch company might try to pull for going sick too much?
Is there a max amount of days you can take sick visa wise?
