With the recent thinking cuts, Claude has not been reading long prompts or supporting documents in my sessions. This has been affecting Opus 4.7 and Sonnet 4.6. What it does is instead of reading the prompt, it makes up what it thinks the prompt is, then uses that for it's output. This is especially bad for supporting documents. It will not even take a peak and make up things about the document. For example, I wanted it to create a practice test based on my notes. Instead of reading them, it made a general mock test for the subject at large. It typically only tries to read once I call it out. Just adding a line in my prompt telling it to read and think isn't enough. Has anyone else had this problem and know how to fix it. thanks.
