12 months on TRT - Time to add E2 blocker?

36M on TRT through a clinic in Australia. Looking for experiences from guys who’ve had similar bloods/symptom patterns, specifically if I should consider adding a low-dose oestrogen blocker, anastrozole (clinic's recommendation based on latest bloodwork).

Started TRT due to:

  • low free testosterone
  • joint pain
  • fatigue
  • brain fog
  • low motivation/libido
  • poor recovery

Protocol progression:

  • Started around 125mg/week test e split 2x weekly
  • Felt strong physically, libido improved, but developed elevated anxiety/agitation
  • Reduced dose progressively
  • Tried 90mg/week and felt flatter: lower libido, less gym drive, lower motivation
  • Now on 105mg/week split 3x weekly (0.14mL 3x/week Primoteston)

Current bloods:

  • Total T: 29.1 nmol/L
  • Free T: 834 pmol/L
  • SHBG: 21
  • E2: 211 pmol/L
  • Hematocrit: 0.48
  • Lipids/glucose/liver all good

E2 timeline:

  • March 2025: E2 376
  • December 2025: E2 295
  • Latest bloods: E2 211

Current symptoms:

  • Libido good
  • Motivation/drive good
  • Anxiety now mostly good
  • Gym performance good
  • No major water retention
  • No nipple sensitivity/gyno symptoms

Only things I still notice occasionally:

  • Maybe taking slightly longer to recover from training sessions
  • Occasional moodiness/emotional sensitivity

Clinic is suggesting introducing low-dose anastrozole because E2 is above target, but I’m hesitant because I’ve read a lot of guys feel worse crashing E2.

Interested to hear from anyone who:

  • naturally runs higher E2 on TRT
  • stayed without an AI long-term
  • or found symptom relief from a very small AI dose without hurting libido/mood.

Not looking for medical advice, just real-world experiences from others.
