I’m exhausted and need a second opinion

Trying to figure out what’s going on.

I’m dealing with recurrent episodes that have a pretty consistent pattern, and I’m hoping someone here has had something similar and gotten answers.

The episodes usually start with small “waves” of a pit-of-the-stomach drop sensation — each wave only lasts a second or two. Over time (sometimes up to an hour) the feeling builds in intensity. My resting heart rate climbs from a baseline in the 70s into the high 90s and stays there, and the elevated HR always tracks with the “weird feeling” coupled.

On top of that buildup, I sometimes get a tachycardia run to 130+ bpm. The run only lasts 2-5 minutes and self-terminates. During the run I feel frequent PVCs layered in, easily 10+ per minute. The pit-drop sensation can persist for over an hour even after the rate comes back down.

Other symptoms during/after:

• Heaviness in my limbs
• Tingling on my head, neck, jaw, and limbs
• No dizziness, no chest pain

No medications, no alcohol

I’ve captured two episodes on a Kardia (1-lead) — both showed sinus rhythm with tachycardia and PVCs (not SVT, not AFib, not flutter). Episodes are infrequent enough that prior Holter/patch monitors haven’t caught one.

I have a mildly dilated left atrium (4.2 cm) on echo and a long history of occasional PVCs. Tonight I ended up in the ER during an episode — electrolytes, anemia panel, thyroid, and chest X-ray all came back normal, 12-lead EKG was clean, but my troponin came back mildly elevated (9) They’re doing serial troponins now. Update: troponin dropped to 7 after an hour and I am going home.

Has anyone had something that looked like this? Sinus tach with PVCs in episodic surges, with a long autonomic-feeling tail and a troponin bump?

Worried about onboarding with BAH being disrupted by possible criminal charge

I am slated to start on the 26th but recently an aggrieved neighbor of mine threatened to slap a criminal charge on me. Maryland allows anyone to go file a criminal charge on anyone at anytime at a district court. They have a long history of filing frivolous cases on others for petty misdemeanors that are always dismissed and never get to trial. I am worried that if they follow through on that and a charge appears on me during my first days that BAH could retract the offer or delay my full onboarding until the case would get thrown out weeks or potentially months later.

Am I being too paranoid about this? As long as I report to security if a charge appears I should be fine, correct?

Freshwater fishing spots for eating-size panfish?

I’d like to take a crack at catch and cook, but lots of places here are heavily fishing pressured. Where could I go and try to catch eating size bluegill and crappie, without needing a boat?

For non-Marylanders, this state is unique in that by law anyone is allowed access to a district court commissioner at anytime of day. When they get there they can file an “application for statement of charges” where anyone can allege anything on the form and if the commissioner (often those with no formal law training) stamps off “probable cause”, it automatically begins a criminal case. Criminal summons and sometimes warrants for arrest are issued without the police, prosecutors, or any other parties being involved. This is solely done based on the word of the complaining party, ex parte, with no requirement of evidence being presented.

You can only imagine how such a process can be abused.
