What kind of flash setup do you use street a style portrait photography?

I'm mostly thinking about my kids. When we're running around at the park or going on hikes or doing other random activities, it can be really hard to get quality exposures on my kids because it's usually the middle of the afternoon so the lighting is harsh, and they're constantly running around so they aren't really interested in posing.

A couple of times I've tried to use an off body flash with a cheap diffusor, and it seemed to work fairly well aside from being really awkward to hold both the camera and flash.

Is there another setup like this that works well? I recently saw some photographers at an indoor event that had a huge bulb style diffusor over their flash (pointed straight up), and it had sort of a mini flash on the camera that was aimed forward, so it was almost like a 2 flash setup

Would something like that be better than just holding a flash in my other hand?
