Severe external hemorrhoids (M/35)

Six weeks ago, I suddenly developed several external hemorrhoids (see photo). They were so painful and I found it difficult to walk or sit. I couldn't poop (until I went to hospital and they said they weren't going to operate as the UK medical system avoids surgery for hemorrhoids where possible [wtf] and I was given laxatives and painkillers). I suffer from pelvic pain/ sphincter muscle spasms so I had anal sphincter botox a week prior which could have caused them, but it was my second time having it and the first time was totally fine (hemorrhoid free). I am now on day 44 and most of them have gone away (leaving some saggy skin and potentially tags), with the exception of one stubborn hemorrhoid which is still relatively large, hard and blue. It is uncomfortable to the touch but not painful (unless prodded). I have since seen another proctologist who simply said to wait and manage diet etc. I am a professional dieter after years of managing my fiber intake, bowel habits, exercise etc - so the fact that this is lingering and potentially long term is ruining my life. It is driving me into a depressive and anxious stage of life. I was considering paying a private specialist to assess and [potentially] do surgery, but at this stage I don't know what to do, think or expect. Any guidance welcomed :-(
