EBT SNAP Deactivated/Need Replacement

My snap card is deactivated due to it being electronically stolen. I called (718)-557-1399/(888)-328-6399 but I wasn’t able to speak with an agent and called ended as a result of deactivated card. They said to contact my case worker, but I do not know who my case worker is.
Idk what my next steps are…thank you in advance
