Can Anyone Help Figure Out What's Wrong With Me?

I'm 21, AFAB, and have been having emergency-room level pain with my periods since I was 12. I've tried the pill, and am currently at a year on Depo-Provera, and both methods worked okay for 3-4 months before ultimately stopping. I cannot use a copper implant (as I know I'll pick it and break it), or an IUD due to vaginismus.

I've still been cramping, albeit at a slightly lesser scale, and am bleeding/spotting/passing clots of tissue every single day. Blood paneling has shown nothing abnormal, including anything involving my thyroid, and with an ultrasound today nothing was immediately seen.

Please help me figure out what's been going on with me. I'm struggling to stay calm and I don't want to be in pain anymore. What other options can I take to figure this out? ​I cannot undergo a pap smear or other invasive procedure without anesthesia due to trauma, and my local office won't do that for me.

Trying to lose weight with a buddy as emotional support (and big Army hates me, lol) and I don't know if this is a safe average to have. The daily goal crono wants me to hit is about 1700 kcal, but im usually below that by a good chunk most days because I don't feel a need to eat more by the end of my work day (I am a retail cake decorator). Is there anything I should be fixing or changing here? I haven't seen any weight loss from this after doing it for two months, either — if anything I've gained 4 pounds off it.​
