Old boss does not like me

Before I decided on PA school I worked as a DSP at the end of the job I was a supervisor on call 28 out of 30 days of the month and not on call the last weekend of the month. My boss asked me to come in on one of those 2 days off and I refused and was fired because of it. However I have over 1000 hours of PCE from that job and 400 ish hours of supervisor expiernce from that job. Should I still include it in my application or start from scratch. I am just scared she will be called to verify the hours and paint a bad picture.

Driving license was suspended

When I was 16 my driving license was suspended for too many speeding tickets in 1 month so my license was suspended for 1 month. Looking through some PA school prerequisites in the additional considerations the back ground information asks about any license suspended or restricted in any way by an instruction state or locality. Does this mean my application would be trash if I was to apply should I pursue another career?
