1.5 years with sciatica, now getting surgery

Hi everyone,

I’ve been dealing with sciatica for about 1.5 years now, 23M. I’ve done pt for every second of it and it did help a bit. I’ve gotten 3 ESI. First and second one worked amazing. Was running and going back to normal life. Got a 3rd one because I was dealing with tremendous back pain for about a week. Long story short after a couple days of the 3rd one, i ended up in the hospital. Left leg was completely numb and pain was very intense. Consistent burning on my Hip, shin and foot which i had never experienced before that 3rd ESI. After a couple months of PT that went away. I was going back to normal but last week I had a flare up. Got an MRI done and Today went to go see the results. Brand new L4-L5 Herniation that was bigger than my original L5-S1. Been dealing with a lot more intense pain in my left right and buttock. Scheduling my MD soon. Does anyone have any advice on what I should do? Continue with PT or go for the surgery. I do eventually want to pursue a career within law enforcement. I was in the process and even passed a state police PT test while dealing with sciatica back in November, had to withdraw from the process because I literally can’t run anymore. Just looking for some advice or anyone experience with an MD
