I need help being better with my Islamic duties because of low motivation and laziness.

Asalamualaykum! I’m a 19 year old and I need some help. I’ll tell you guys my day to day schedule when I’m on summer break and then you guys can tell me how I can fix my days up to not be so sluggish. I always struggle with ups and downs of my iman and specifically getting back into the routine of doing my Islamic duties.
5:30 am pray fajr, sleep by 6 after reading manzil, wake up by 11-11:30 pm, eat breakfast, do all of my chores, get ready for the day by 1:30, then get ready to pray zuhr and finish praying by 2:10. Now here is where I want to read Quran or do tasbih, but I sit on my phone until 3:45 and then watch a tv show from there until 5ish. I’ll maybe go spend time with my family until 6:30 where I get ready to pray asr and sometimes delay it until 7:15 but most of the time I read it by 6:40. Then I’ll scroll on my phone until 8 or watch my tv show AGAIN. then my sister comes home from Quran class and we play volleyball itside or go walking with my cousins. By 8:50-9 I’m back inside the house, praying maghrib. Then I eat dinner since my little brother gets back from Quran class late by 9ish. Then I clean the kitchen and dishes, drive to the masjid sometimes or get ready for isha. If I stay at home to pray isha then I’ll scroll in tiktok until 11 when isha starts at 10:20-10:30. Then when I planned to read surah mulk and waaqiyah I end up just praying isha and 2 nafl, some istaghfirullah and sleeping. Sometimes I even scroll until 1-2 am. Then the cycle repeats.
How do I create a routine that brings barakah in my life when I’ve tried restricting my phone usage but the longer I put it away for, the longer I use it later. I’d be fine for 2 days max and then the next week I’m back to this horrible routine. ANYTHING WILL HELP PLEASE. Jazakallah!!
