Is this Hypoxylon Canker?

These were all sent to me, if any more are required per the guidelines I will ask for more pictures and reupload.
Many branches among the canopy are dead or dying, bark is starting to come off.
-Location: Upstate New York
-Yes it can survive this area
-It was planted at least 30 years ago
-It is getting full sun, it has enough space between the trees next to it
-Only watering during extreme heat in summer
-Not a container/ B&B tree
-Unsure of procedure when planting
-Cannot see root flare
-No plastic or landscape fabric
-No heavy construction, heavy traffic, or digging in recent years, no chemical application
-Some branches broke off during heavy storms, visible decay at base of tree.
