new grad multiple potential jobs

Hi! I graduated in December and have been applying to jobs since November. I'm looking in multiple states. I currently have 3 potential opportunities in different stages of the hiring process.

  1. A LTC consulting company. I know that's usually not a great starting place for a new grad but this company actually prefers hiring new grads and seems like I would have adequate training and support. They are in need of hiring someone asap it seems. I have had a phone screen and a video interview. I go to shadow someone this week. I would likely have to move for the position (it's over an hour away, more with traffic and I would possibly need to go to multiple different facilities.) I have a feeling I would get an offer for this position first, but I am least interested in this position.

  2. Neurology with a well known teaching hospital. I am in the process of having a phone call this week and then scheduling a site visit sometime soon. They are also in need of hiring fairly soonish, and they did hint at ideally wanting me to sign by early/mid June since onboarding will take 3 months. I would not have to move for this position and would prefer it over LTC.

  3. Urology which I have had 2 phone calls and looking to schedule an in person interview within the coming weeks. I am most interested in this position, as I would get OR time; however, it would require a cross-country move and being states away from my significant other (who btw is incredibly supportive of any decision I make... and we have done long distance before so I'm not worried, but obviously this is not ideal.) I do not know the salary yet for this position, but I do know that I would have fantastic support as a new grad. The docs love hiring new grads and wouldn't expect me seeing patients on my own until I was fully comfortable. They are interviewing ~10 other applicants and want to make a decision within the next few weeks so I honestly don't know how great my odds are. I think my edge here is I have connections with the area/went to PA school nearby.

With student loan payments coming up, I NEED a job. My interest of the 3 in order are: 1) urology, 2) neurology, and 3) LTC.

If I get the LTC offer first, before hearing back from the other positions, how do I go about it? Do I ask for an extension on the deadline to accept an offer in hopes I'll hear back from one of the other two places within that time?... and do I disclose that I am in the hiring process elsewhere? Or do I just take it since I need to start making money and gaining experience as a provider?

I know I'm putting the cart before the horse here but I would like to have somewhat of a game plan going into things.
