Hi is it possible to do medical electives or observership on ESTA in a private clinic?

What to say at the customs control when entering

I'm a final year med student from Europe and got offered electives but in a private clinic, will it be possible to do this on ESTA or they only let go people who are doing electives at hospitals affiliated with universities?

I have an invitation letter.
