Treatment plans in private practice - How detailed are you making them?

So how detailed are we making our treatment plans? Do you take the time to make the goals measurable/quantifiable? Do you set target dates for each of the short term goals?

At one time, I made mine really simple. Then I as I got more concerned about the potential for an audit, I started including dates and measurable targets. Then I started seeing the simple plans that most of my colleagues write and have started matching my style more closely to those. Now the practice I contract with is being audited, and I'm wondering if it wasn't worth it to do the complicated plans after all. How do you all handle this? How much do the treatment plans figure into how we fare in an audit?

Are the dyes used in egg cartons and cardboard toxic at all for garden use?

pretty much what the title says.We don’t put much cardboard in our compost, but use a lot of eggs and use the egg cartons as liners for our compost can. My wife asked this morning if there’s any concern from the dies they use. (Food Lion just switched their egg carton design so it now has a bluish tint.)
