Should I create an accessibility passport for ongoing health issue?

Here is the situation.

I am a type 2 diabetic not on insulin. It is relatively well managed but the medications that I take to manage it cause some fairly severe side effects. I get periodic bouts of severe stomach issues which means I have to stick close to a bathroom. Also, if it hits at night I don't get a lot of sleep and I feel pretty rough the next day. Also, being immune compromised I tend to pick up bugs fairly easily.

This was an issue for a previous manager before the pandemic as I took a lot of leave and generally burned through my vacation taking a day here and there. It came to a point where she was asking for a medical note every time I was ill. She also mentioned that she had some health issues that she had "fixed" and that I should do the same. She also ruined my reputation within the department and opportunities were missed as a result. Thankfully she left and I had a bit of a reset with the new manager.

Then the pandemic hit. Working from home was better and I was able to build my sick leave bank since I could still work from home if my stomach was an issue. I took a lot less leave.

Now that we are back in office, I am taking an average of 2 days per month off. They tend to be on days I am meant to be in office as the issue is the need to be immediately close to a bathroom.

I have a pretty good manager, but he recently raised this as an issue. He is likely getting pressure from his higher ups, as this is likely skewing the in office presence numbers for our team. He asked if there is anything that the organization could do to help.

I am not really looking for any accommodation other than not giving me a hard time when I need to take a day off. Should I fill out the passport in this regard? Looking for thoughts.
