5 months late period, now extremely heavy bleeding

Hi all. With diet changes, working out and supplements, I had managed to get my period fairly regular last year - I have had a PCOS diagnosis since I was 15, and I'm now 29. At most in the past, I've had my period be as late as a month at most, but this time it was 5 months late and I got it 2 days ago. I had an ultrasound scheduled for tomorrow after finally getting one prescribed after many "it's normal/use birth control" conversations with doctors - which I cancelled as soon as I got my period.. i realize now, dumdum on my part. But now I'm bleeding a bit too much and idk when to head to the ER. I'm a heavy bleeder in general, but for the last 2 days I've been having my menstrual cup overflow in about 2 hours - this would normally take me 6-7 hours only during peak days and never continuously. Am I freaking out for no reason? I do feel very weak and dizzy.
