Stage IV gastric cancer with peritoneal metastases – anyone had conversion surgery/HIPEC after responding to chemo?

Hi everyone, I’m 57 with Stage IV gastric adenocarcinoma with peritoneal/omental spread and ascites. I’m on oxaliplatin, capecitabine and trastuzumab (HER2+), and have had a partial response so far.

My UK oncologist currently does not recommend surgery due to peritoneal disease. I’m looking to hear from others with primary gastric cancer and peritoneal metastases who were reconsidered for surgery after responding to treatment.

Has anyone:

• Initially deemed inoperable due to peritoneal spread, then had conversion surgery?
• Had repeat staging laparoscopy/PCI after chemo?
• Had gastrectomy with CRS and/or HIPEC?
• Been assessed at specialist centres in the UK, US, Japan, etc.?
• Been declined at one centre but accepted elsewhere?

I understand surgery isn’t suitable for everyone and I’m not seeking to override my oncologist—just exploring whether a specialist second opinion is worth pursuing if my response continues.

I’m especially interested in cases where the primary was gastric cancer with peritoneal/omental metastases.
