Question Regarding PSA Amendment of Report of Birth – Adding Father’s Name/Surname
Hi everyone. I would like to ask if anyone here has experience with PSA amendments, particularly for a **Report of Birth registered abroad**.
The children were born in the **UAE**, and their Report of Birth was subsequently registered with the Philippine Statistics Authority (PSA). The children are currently using their **mother’s surname**, and the **father’s information was left blank**on the Report of Birth.
The father is a **foreigner**, and he would now like to have his name added to the children’s Report of Birth and, if possible, have the children use his surname.
I already contacted the **DFA**, and they advised me that once the Report of Birth is registered with the PSA, the matter will be processed directly by the PSA.
What are the possible requirements, procedures and can I process it in any PSA branch?
The PSA is not answering my email, text and can’t contact them as well via Telephone.
