1 year post-Bankart repair – shoulder/clavicle complex feels unstable, shoulder sits lower and feels like it separates when arm elevates. Looking for ideas.

Hi everyone,
I had dislocations of left shoulder (right in photo) and had Arthroscopic Bankart repair a year ago.
Prior to surgery I also had a deep massage injury around the upper back/scapular region which caused immediate neurological symptoms and worsening shoulder mechanics.
Since surgery, I continue to have significant dysfunction despite extensive rehab, and compensation patterns, 2 months ago I had a myofascial massage that’s causing this separation as you can see in my left shoulder (right in the photos) when it elevates with lots of clunking.

Main symptoms:
Feeling that the shoulder girdle “disconnects” from the trunk during elevation like in the photo.
Frequent clunking/clicking around the shoulder, clavicle and scapular region.
Sensation that the humeral head translates or separates when the arm moves in points.
Scapular dyskinesis type2/3
Neck and upper trap compensation.
Subjective weakness and poor proprioception on the left side.
Tongue sensations and allodynia

Does this look more like scapular dyskinesis?
Could this be sternoclavicular (SC) joint or AC joint dysfunction?
Does anyone see evidence of excessive scapular elevation/downward rotation?
Could persistent neuromuscular inhibition or long thoracic/spinal accessory nerve issues create this appearance?
Has anyone seen a similar case after instability surgery?
