pain in the inner/front side of my elbow around the distal biceps tendon area

i honestly don’t know what’s wrong with my right arm anymore and it’s driving me insane.

some backstory: since childhood i’ve been doing martial arts, pull-ups, calisthenics, dumbbell curls, grip training etc. around 2023 i started feeling pain in my right arm around the inner/front side of the elbow, kind of near the distal biceps tendon area. not exactly in the biceps muscle itself, more like slightly above the elbow on the front-inner side.

every time the pain calmed down a bit, i went back to training.

i went to doctors multiple times. they did x-rays and ultrasound scans and found basically nothing. i was told to rest for a week and it would go away. but i’ve already had many “rest weeks” before and the problem always came back.

then sometime in 2025 i completely ignored the pain and kept training through it hard. i was doing like 50 pull-ups daily in 3 sets, lots of push-ups, resistance bands, grip trainers etc. the pain never disappeared and only kept getting worse.

eventually it got so bad that i literally couldn’t put my hand into my pocket without pain or bend my arm properly. i started walking around with my arm slightly bent because extending/flexing it hurt too much. one doctor even put my arm in a cast for a month because he thought there could be a distal biceps tear if i didn’t stop.

after the cast i obviously stopped going crazy with training, but the weirdest part is that even after almost a year with basically no real training, nothing changed.

sometimes i do small stuff like 30 push-ups. today i literally did 5 kg dumbbell curls for 8 reps x3 sets, which should be absolutely nothing for a guy my age and size. my left arm feels completely normal, but my right arm immediately started hurting again from THAT alone.

that’s what confuses me the most. why is my right arm so insanely sensitive now? how can it still react this badly after such a long break?

sport is basically my whole life and this situation is mentally destroying me. i genuinely don’t know what to do anymore.

has anyone experienced something similar? chronic distal biceps tendinopathy? brachialis injury? nerve issue? medial elbow problem? i’d appreciate literally any advice or similar experiences.
