Okay so there’s this famous step-up but isn’t it for adjusting the treatment in someone who’s already been on treatment? If so, how to decide on initial treatment?
I read that the treatment goal should be <2x/week daytime symptoms and <2x per month night time symptoms. So let’s say a patient has 3-5x day time symptoms per week, we start them on PRN ICS-Formoterol. If the pt still experiences symptoms at same frequency but being relieved from inhaler, would this pt still need a step-up treatment to maintainance therapy?

Recently we had an awesome post from a cool human who went through uworld multiple times. I know the conventional advice is to go through as many new questions as possible. But lately I am feeling that the consolidation one might get from going through similar concepts multiple times can actually be valuable, especially if they are spaced.

Also an interesting idea that I have learnt from the post is that each question tests a learning objective in actual exam. This actually makes sense in retrospect. What do you guys think?

What can we objectively gain from multiple passes? Are we underestimating it?
