Stage 4 about to get off chemo and transition to surgery/HAI Pump

In late December after having a colonoscopy and a battery of CT MRI scans. It was clear that I had over 35 lesions in my liver. I'm based out of Ottawa and the Ottawa Cancer clinic gave me roughly 2 years to live. I'm 48, Married with three kids. It was devastating to say the least.

If you check my profile, you'll see that I'm an engineer that designs lots of cool stuff. So that's the hat I put on when I got this news. I told myself the number One thing I need to do is understand what I have and try to get as much information as possible. I also researched lots of different supplements but I don't want to talk about those because I don't think they had a large effect on my outcome. But if you're interested in the stuff I took, please shoot me a line.

The first thing I did was fire my first oncologist. They told us their style of oncology is to be as reserved as possible and not try to do anything radical. This is exactly what we didn't want . They were dismissive with a lot of our questions about other treatments options and didn't want to offer us a second opinion in Toronto. They eventually relented and we got a second opinion which I'll speak to later. Our new oncologist is fabulous and has been extremely helpful working with us

We had a fair amount of other issues with the Ottawa hospital. It took them about 2 months to figure out I needed to be on chemo which is a long time in the Cancer world. The surgeon we spoke to also was misinformed with a lot of the newer technologies.

This next thing we did is we got the foundation one testing on the primary site biopsy. It was about $2,000 Canadian but was well worth it. It basically stated I had a KRas mutation. This became very helpful in understanding the future response I would see under systemic chemo.

I've gone through 12 cycles of chemo so far and my last CT showed my liver is clean. The primary site is no longer visible on scans. It was actually fairly clear after four cycles with only a few small areas of hypodensity but no measurable lesions. My CEA dropped from over 10 to 2 as well.

I had a pretty rare ( in a good way) cancer in that I had only one mutation and TMB was very low. I was also MSS Stable. I have read that this type of cancer is highly susceptible to standard chemo because it lacks other mutations that inhibit cell death. The kras mutation is only a driver.

My second opinion in Toronto gave me a much more optimistic outlook. They told me the pump was a path to cure and this was even before the 4th cycle scans showing an almost complete response.

So I wait a few months to clear out the chemo and I get my colon removed, resection if needed,.and the pump. I'll wear the pump for about 6 months to a year and it doesn't have any of the side effects like standard chemo. It floods the liver only and will hopefully kill any remaining sleeper cells.

If everything goes well I will be in remission.

My newest cutest Power Supply Design

This little guy is about 12" x 5" x 5" and is an AC/DC Power Supply. It takes in a weirdo AC input that isn't PFC friendly and outputs combined 250W across 34V/24V/12V/5V rails. It's about 93% efficiency at the moment but I hope to tweak things a bit to get higher. It provides telemetry over CANBus using CANOpen layer.

I've been doing AC RMS Power calculations way to hard

I design power supply modules and customers like a rough value of AC Input Power to be transmitted over CANBUS or I2C. It doesn't need to be exact but in the ballpark. I've always done these RMS calculations the hard way. Here's the workflow

  1. Sample voltage and current

  2. Store in memory array as a weird floating point type

  3. Use a hardware cordic function to calculate the sqrt

  4. Multiply V & I

  5. I typically assume high PF and prestine waves because I'm using a PFC boost converter.

The cordic runs a Newton's method solution that actually takes a fair amount of cycles. The conversion to an odd floating point representation always causes me consternation.

That was the past, moving forward to today I'm building a new type of power supply for a niche application. It doesn't allow the use of a PFC due to stability concerns because the transformer is ancient tech. This means I need to figure out power factor so I can calculate real power. My primary side microcontroller is pretty heavily burden so I need to keep calculations as simple as possible. So attached is my algorithm that calculates real and apparent power and it never touches a sqrt. It's basically just an average energy calculation.

I assume the current waveform is stable across different loads or in other words the RMS multiplier is constant.

So let me know if I'm an idiot that I haven't realized this for over 20 years.

Modified citrus pectin

I'm on chemo right now and I started taking MCP about four cycles ago and it's been quite helpful with minimizing recovery time. It used to take about a week to feel better, but now it's a few days after the infusion is done . MCP is a fairly new supplement and it has shown some efficacy in testing for reducing toxicity of the chemo and some other benefits.

MCP is derived from the food product pectin so it's fairly safe with little side effects, but I would always talk to your oncologist about interactions.

I've also been taking lumbrokinase pre chemo and occasionally during the off cycles. It's a powerful anti-fibronic and has shown efficacy for improving the response of chemo. This is especially true with me as four cycles all my metastasis disappeared and my CEA dropped to 2. Please message me if you're interested, I would be happy to explain how it works and the risks. Should I build a custom rtos

I design power supplies and timing is very important. I have tasks thst go off at 1 millisecond, 10 milliseconds and 100 milliseconds. Right now I use threadx and use timers to call the various tasks. This process always seems overly complicated as I would much rather have an RTOS that just allows you to define task times at the getgo.

For instance, you would initialize a task with a particular time interval and that's it.

Is it reasonable for me to make this custom rtos or is the Simplicity and possible code size reduction arbitrary at best. I am using an stm32 G4 running at 170 MHz so it's not like I'm band limited.
