Hello po, i am on the process of filling out all the forms needed po para sa enrollment and may mga few clarifications lang po ako. Sana po masagot hehe.

  1. Should I accomplish the Permit to Enroll form myself, or will it be completed by the admissions staff?

  2. Since our graduation is scheduled in June, what should I indicate in the Student Record Form for the number of absences and tardiness in Grade 12? May I write “to be followed” or “as of latest grading period”?

  3. Should all entries in the forms be written in capital letters?
