My father has 100% service related disability. He recently broke his hip and is in a skilled nursing facility that is trying to discharge him, but he can't even walk again despite making progress. My mother has her own health issues and has limited energy. She has medical power of attorney. I live far away and am trying to make phone calls to help.

After I got a lot of hangups at different VA numbers, I got through to someone who said I can't talk to them because I'm not authorized. Some of the research says form 10-5345, which is the release of health information, but I also want to leave messages with his health care team. I looked at form 21-0845 but that says it is only for one person and I don't know if submitting that will make my mom ineligible to talk about his benefits.

Does anyone have experience being a secondary point of contact besides the veteran and one other person? What forms are needed?
