Advice on best approach for US/UK website structure

Hello,
Our audience is predominantly based in the UK and the US, with additional visitors from Canada and Europe.
 
As each region has slightly different healthcare systems, terminology, pricing, and ways of working, we want to provide the most appropriate content, support, training, and pricing for each audience.
 
Previously, we attempted to use visitors’ IP addresses to automatically direct users to the correct regional pricing and content. However, this proved unreliable within some US hospital networks, where location services or redirects appear to be blocked. As a result, some US users were shown UK pricing and left the website.
 
We would therefore appreciate your advice on the most reliable long-term approach.
The options we are considering are:
• A landing page where users select their country or region before being directed to the relevant version of the site
• A multisite setup with separate UK and US websites
• A structure such as:
example.com/uk/
example.com/us/
with region-specific pages including:
example.com/uk/training
example.com/us/training
 
We would also like each region to have its own navigation/menu structure where appropriate, to avoid confusion for users.
 
One of our main concerns is SEO and preserving existing links. For example, we already have public URLs such as:
example.com/training
We would not want existing links, rankings, or indexed pages to break.
With this in mind, would it be better to:
• Keep pages such as example.com/training as global landing pages where users select UK or US content
or
• Implement redirects to regional versions in a way that remains reliable for hospital networks and does not negatively affect SEO?
 
We would be grateful for your recommendation on the best overall approach, including any implications for:
• SEO and indexing
• Redirect handling
• Hosting and backups
• Storage requirements
• Ongoing maintenance and content management
