Should I stop using gauzes? (Post-fistulotomy, now new anal fissure)

Had a fistulotomy to remove a simple fistula in February.

Wore gauzes basically non-stop because there was a lot of drainage.

My proctologist says the wound has closed, but there is still some discharge to be expected.

Unfortunately, since last week I have a new anal fissure on the scar tissue. It bleeds after I go to the toilet and it hurts. Doctor said I should use nifedipin and it shouldnt be an issue.

Whenever I use nifedipin and then a gauze, I find it all seems super messy and humid. I have been wondering if me wearing a gauze at all times might have facilitated and now irritates the new fissure - i put the gauze between my ass cheeks and I can often feel it push towards the wound. It wasnt really an issue before, but now I think it might be.

Would you suggest I should stop using gauze completely? Sometimes I have brownish (probably stool mixed with wound exudate) lines/stains on it, but I think the gauze might be absorbing the stool through the anus. I wonder if it would be less messy without a gauze. Maybe I could use simply a pad in my underwear instead?

Grateful for any advice!
