high school math teachers - why do so many students fall behind? and is there anything that actually helps them?

back in high school, i really struggled with math. but it seems like the amount of students falling behind today is getting much worse.

for those of you who teach high school math (algebra 1/2, geometry, trig, calc), how many of your students would you say fall behind each year? and how well do things like tutoring, extra help, or online tools actually work? (if at all)
