Prefab Home Moisture Issue

Hi everyone,

I recently inspected a prefabricated home and found no significant issues. About a month later I received a call from the home buyer stating that there was moisture damage all over the house. I took a second trip to the property to try and get some answers for my customer. Sure enough, some of the walls had turned into bread pudding. I was in a state of near disbelief. I went back onto the metal roof and found no apparent defects. There is no attic access therefore I was unable to completely verify that there was no roof leak, even though I saw no stains on the ceiling. I went under the house and found no suspect plumbing. The only thing I can think of is when I initially inspected the house, the HVAC return filter was completely saturated and the customer still had not changed the filter. (It doesn't look like it's been changed in over a year). My moisturizer meter did pick up high levels of moisture around the ducts and registers but that just seems bizarre that the AC could do that much damage to the interior walls. Has anyone else had this experience? I'm at a loss here.

Is This A Defect?

I did an inspection this morning and came across this in the attic. It's the range exhaust hood duct and it appears to be held together by duct tape. Unsightly, yes. But is it a defect? I'm not 100%. I could not verify if it was held together by any other means. The other picture is a picture of my cat for no reason.

Is This A Defect?

I found a few gusset plates secured with nails. Was this done during manufacturing of the truss', or was it likely altered during construction of the home? I can't imagine this was how it came out of production.
