Hi all. I’ve had 2 RFEs, first time was about missing IPV but I have had 4 shots of OPV when I was a child in my home country. I refuse to argue with USCIS and got the IPV, second time was because the incorrect filling of IPV/OPV by my civil surgeon.

Questions:

1. How to correct fill the IPV/OPV row? Currently have all dates listed + both IPV and OPV marked, but it’s an issue for USCIS because they are not specified. Should my civil surgeon only checkmark IPV + list date + mark complete? My civil surgeon told me she thinks my 4 shots of OPV are enough and I never have to get IPV so she will remove the IPV dose and add notes to explain.
2. RFE is saying the submitted Form I-693 was certified by the civil surgeon on July 30, 2025, prior to the administration of all age-appropriate vaccinations were administered and properly documented, does it mean I have to redo my physical exam? Or can my civil surgeon just simply resign the form with current date?
