Lower back pain that feels menstrual

I have recurring pain in my lower back that feels so much like menstrual pain that I run and check to see if I have started my period. If I were pregnant I would think it's early labor, it feels just like that. It feels like it's in my middle lower back and radiates from there. I have had ultrasounds to rule out any obvious gyn issues. I have tried muscle relaxers and NSAIDs, they do nothing (which is where it's different from my actual period cramps). I have tried stretching and resting and not resting and changing positions and a basic back brace. The only thing that seems to help is heat and certain antihistamines. The antihistamines that work are of course the ones that knock me out (I'm so sensitive to anything that might cause drowsiness) so I can only take those early in the evening and usually at a half dose so I can function the next day. I have zero clue what triggers it and it will last a few hours to days.

Does anyone else have this pain? Success at treating it? Ideas on potential cause? I am hEDS/POTS, no MCAS or other physical dx at this point.
